We offer a comprehensive and competitive benefits package that includes insurance, vacation and time off, competitive pay, 401Ks, opportunities for continuing education and discounts on hospital programs and services.

Benefit Who Pays When Eligible Benefit Received

Group Health Insurance

CCH/staff

First of the month, following date of hire (must be scheduled to work 24+ hrs per week)

Three options with different levels of deductibles, drug cards and co-pays. Midlands Choice PPO. Premiums - Wellness rate if employee completes wellness program criteria each year or regular rate if they do not complete wellness program criteria.

CCH/staff

Eligible within the first 30 days of employment or quarterly thereafter for employee contribution

Staff contributes between 1% and 100% of income on a tax-sheltered basis. CCH match up to 4% max, after 12 mos, 1,000 hrs. worked, 21 yrs. old. Match schedule: 1-7 years of service - 4% match; 8-15 years of service - 5% match; 16+ years of service - 7% match. Your contribution will increase by 1% each year until it reaches 12%. Opt-out option available. Roth is an after-tax contribution.

Group Basic Life Insurance
CCH
First of the month, following date of hire (must be scheduled to work 36+ hrs per week) Base life insurance amount equal to one time the annual salary. Term type insurance.

Group Long-Term Disability Insurance

CCH

First of the month, following date of hire (must be scheduled to work 36+ hrs per week).

After 30-day wait, plan pays 66 2/3% of base salary, 3/12 Pre-Ex: Any claim incurred w/in first 12 months after eff. date would be denied if a condition exists w/in 3 months prior to the eff. date.  

Voluntary Life Insurance

Staff

First of the month, following date of hire (must be scheduled to work 24+ hrs per week).

Term type insurance for employee, spouse, dependent children. Guaranteed amount if enrolled during first 30 days of eligibility.

Dental/Vision Insurance
Staff
First of the month, following date of hire (must be scheduled to work 24+ hrs per week) Preventive and routine care benefit.
Flexible Spending Accounts
Staff
Within the first 30 days of employment (must be scheduled to work 24+ hrs per week) Tax-free deductions for payment of dependent care or un-reimbursed medical expenses.

Health Savings Accounts

Staff

Must participate in the CCH High Deductible Health Insurance Plan within 30 days of DOH (must be scheduled to work 24+ hrs per week)

Tax-free deductions for payment of un-reimbursed medical expenses. Annual contribution cannot exceed annual deductible. *$25 employer contribution each month. *Employee must contribute minimum of $25/per pay period to be eligible for employer contribution.

Aflac Insurance

Staff

First of the month, following date of hire (must be scheduled to work 24+ hrs per week)

Tax-free deductions for Critical Illness or Personal Accident Insurance. Benefits paid in addition to other insurance plans you may have.

Wellness Program

Upon employment

Improve employee health by education, engagement and healthier lifestyle. Complete criteria laid out each year for the program.

Paid Time Off (PTO) .085 x # hrs/PP

CCH

Full-time/Parttime status only

Available immediately as accrued. Rate increases after completion of 7 and 15 years of service. 

Shift Differential

Incentive for working between 6 p.m. and 6 a.m., 7 days/week. 

Weekend Differential

Incentive for hours worked between 6 p.m. Friday and 6 a.m. Monday. 

On Call/Call Pay

Incentive for taking call.

Call Back

Upon employment, not all staff eligible

Incentive for being called back to work while on call.

Low Census

CCH

Upon employment, $3/hour, PRN staff not eligible

Compensation for scheduled work hours missed as the result of a decrease in work/case loads caused by lower than expected pt. census.

Emergent/Extra

Upon employment

Incentive for working an unscheduled/unexpected shift caused by an emergent situation.

Referral Bonus

CCH

Upon employment of referred applicant

$1,000 or $2,500 depending on the position recruited.

Holiday Pay

CCH

Upon employment

Incentive (time and ½) for working hours between  midnight and 11:59 p.m. of the following holidays: Memorial Day, Independence Day, Labor Day and Thanksgiving Day. Time and ½ for working hours between 6 p.m. Eve before and 11:59 p.m. day of New Year’s Day & Christmas Day.

“EAP” is a short-term counseling service to help you and your family work through problems/conflicts and assist in legal and financial services. 
Funeral Time
CCH
Upon employment. Temporary, PRN and PT employees scheduled to work less than 24 hours per week are not eligible for paid Bereavement Leave. Time off with pay may be granted for bereavement.
Jury Duty
CCH
Upon employment. CCH will pay regular base wages for scheduled days missed due to jury duty. Employee keeps stipend from the court.

Pharmacy Discounts for OTC medications

Charges based upon cost plus 10%.

Cafeteria Discounts & Payroll Deduction

CCH/Staff

Upon employment

20% discount; payroll deduction for purchases in hospital cafeteria.

Tuition Reimbursement

CCH/Staff

After 3 months of employment. (must be scheduled to work 24+ hrs per week)

75%-100% reimbursement for college courses or certification up to $3,000 per calendar year.

Continuing Education

CCH/Staff

Upon employment (must be scheduled to work 24+ hrs per week)

Per situation, CCH annually pays 100% for required programs; up to $200 annually or $400 biannually.

Direct Deposit

Paychecks go directly to your bank account on payday. 26 pay periods per year; every other Friday. 

Staff Health Services

Upon employment.

Provides for initial and continuing tests and vaccinations.

Worker’s Compensation

CCH

Upon employment

Medical and income benefit for staff who suffers a job-related injury or illness.

Dry Cleaners

Staff

Upon employment

Centralized drop-off and pick-up within our hospital with Prestige Dry Cleaners (Second floor across from unit secretary desk).

College Savings Plan (NEST)

Upon employment

Payroll deduction available for those who want to invest in their child’s future. 

YMCA Discount & Payroll Deduction

Staff

Upon employment or January 1st of every year. (Must be scheduled to work 24+ hrs per week to be eligible for additional cash stipend.)

Discounted membership through payroll deduction; $150 cash stipend every 6 months if 24+ hours/week (paid in July and January).

Exercise Program in Cardiac Rehab

Upon employment. Open enrollment December of each year

Open gym concept for employees/spouses. Gym is open 24/7. $5/month payroll deduction.

Child Care Center

Staff

Upon employment/availability

Licensed for 105 children between ages of 6 weeks to 13 years. Open 5 a.m.-7:30 p.m. Monday-Friday. Payroll deduction available for full-status CCH employees.
